The above named registrant has paid the applicable fee to register for the identified season at the Registrations club noted above and understands that this fee is nonrefundable once the player has been assigned to a team. A $50 administration fee will be removed from any refunds payable prior to the start of the season and if requested within 30 days from registration date.

Registrations recommends that all participants wear a helmet, gloves, elbow pads, Jock ⁄ Jill strap and shin pads and all players accept that they are in control of and responsible for their equipment choices. All participants under the age of 18 must wear a CSA approved helmet with full face mask, and proper hockey gloves. Jock ⁄ Jill strap and shin pads not mandatory but are recommended. I hereby grant permission for Registrations to use and publish any photographs taken during ball hockey events of me ⁄ my child, or in which I ⁄ they may be included for editorial trade, advertising and ⁄ or any purpose in any manner and medium and to alter the same without restriction.

HOLD HARMLESS I the applicant (and We ⁄ I the parent's of the applicant if under 18), hereby give my ⁄ our approval to my ⁄ his ⁄ her participation in any and all league activities during the current season. I ⁄ We assume all risks and hazards incidental to such participation, including transportation to and from the activities and I ⁄ We hereby waive, release, and absolve indemnity and agree to hold harmless the supervisors, officials, sponsors, organizers and corporate owner's of the premises and league. Also persons transporting Me ⁄ My ⁄ Our child to or from activities for any claim arising out of an injury to Me ⁄ My ⁄ Our child.
I hereby agree to comply with all City ⁄ Township by-law  ⁄ regulations as outlined within all affected properties.
